Subject Access request data breaches are rare, and the Trust report these episodes internally and they are reported through the Trust’s Information Governance Steering Group. All episodes are managed to rectify the error in full.

Please be advised none of the data breaches reported externally were Subject Access Request related, and we will not be reporting the breach of your information, as it does not meet the threshold for reporting to the ICO. It has been managed within the Trust as part of our Data Protection breach process and will be reported through the Trust’s Steering Group, which is chaired by a board director.
