1. Gestational Diabetes Mellitus Screening Methodology: Please provide details on the current methods used by your Trust to screen for Gestational Diabetes Mellitus. 75g OGTT

2. Timing of Screening: At which gestational week do the majority of pregnant women undergo GDM screening within your Trust?

28 Weeks

16 Weeks if previous GDM

3. Early Screening for High-Risk Pregnancies: Do you conduct GDM screening earlier for pregnant women deemed to be at very high risk? If so, please specify the criteria for determining high risk and the gestational week at which such screenings are conducted. Previous GDM – 16 weeks

4. Administration of OGTT:  Who is responsible for administering the OGTT within your Trust? Additionally, where is the OGTT typically conducted (e.g., hospital clinic, community health center)? Day Assessment Unit

5. Maternity-Related Risks Recorded in the Trust Risk Log: Please disclose which maternity-related risks are logged in the Trust’s risk register or risk log.

The Trust has included the risks related to maternal medicine/diabetes care that are on the Directorate Risk Register:

• Managing complex pregnancies – maternal medicine

‘There is a risk that Worcester Acute Hospitals NHS Trust will not be able to provide the expected level of care required to manage women with complex pregnancies based on the Okenden report. This is because no formal agreed pathway for joined up clinics has been agreed, this may result in inconsistent care provided to women’

• Only one Preterm prevention clinic per week

‘There is only one pre-term prevention clinic across the three sites with one consultant running these clinics.

This puts pressure on the service and may result in women not being seen in a timely fashion.

When the consultant is away on leave the clinic often needs to be rescheduled to another time.

All of the clinics are help at Worcester and some women from elsewhere in the County are reluctant to travel to Worcester to attend’

• Booking for pregnancy care by 12+6 weeks’ gestation

‘There is a risk that if women are booked beyond 12+6 weeks gestation, there will be missed opportunities for appropriate referral to the correct antenatal pathway which may result in patient harm.
The reasons for this are varied and include staffing challenges, access issues and process issues’
