Request:

Employment of Clinical Roles:

Information on whether Worcestershire Acute Hospitals NHS Trust currently employs the following clinical roles and the total number of staff employed for each role.

Categorised Patient Harm Events:

The number of patient harm events categorised by the level of harm (as defined by NHS England) for each clinical role between 1st January 2018 to 31st December 2023.

Definition of Harm Categories: The NHS England harm scale categorises patient safety incidents into the following levels:

No Harm: No injury or harm occurred to the patient.

Low Harm: The patient required extra observation or minor treatment.

Moderate Harm: The patient required further treatment or experienced an extended hospital stay but did not suffer permanent harm.

Severe Harm: The patient experienced permanent or long-term harm.

Death: The incident contributed to the patient’s death.

Note: If your Trust does not categorise incidents according to the NHS England harm scale, please provide the total number of patient harm incidents from 1st January 2018 to 31st December 2023, categorised by role without specifying the level of harm.

Identification of Harm Events:

The number of harm events identified through different methods (e.g., internal Trust investigations, Coroner’s investigations, regulatory bodies, etc.).

Incidents generally occur within a given ward or area, but an untoward event such as a fall can’t be attributed to a doctor or nurse etc.
