Rapid mast cell degranulation during an anaphylactic reaction results in an immediate rise of serum tryptase levels within 1-2 hours. This reaches a peak at around 6 hours and returns to baseline by 24 hours.In order to reflect this please take 1 clotted blood sample as soon as possible after the onset of symptoms, and a second sample within 4 hours. A baseline sample should ideally be taken 24 hours after the reaction.

Investigation of anaphylactic reactions requires 3 serial samples: Initial sample as soon as possible after symptoms started, Second sample at 1-2h post symptoms, Third sample at least 24h post.
