Elderly Care

We provide a dedicated elderly care service across our Trust, helping patients return to either their own homes or care homes.  We work as a team involving doctors, nurses, pharmacists, dieticians, occupational therapists, physiotherapists and speech and language therapists to get the best results possible for each individual.

We provide a number of specialist services including a group to help those patients who have experienced a fall, patients with dementia and end of life care.
